Underquilt vs Sleeping Pad: Solving Cold-Butt Syndrome
Nearly every new hammock camper runs into the same surprise on their first cool-weather trip: they're plenty warm on top, wrapped in a good sleeping bag, and still shivering from below. The nickname for this among hammock campers is "cold-butt syndrome," and it isn't a mystery once you understand what's actually happening underneath you.
Why a sleeping bag alone fails in a hammock
A sleeping bag insulates by trapping air in its loft — the fluffed-up layer of down or synthetic fill. On the ground, that loft compresses under your body weight, but the ground itself is a relatively poor conductor, so the loss is tolerable. In a hammock, your body weight compresses the insulation directly beneath you against thin fabric with cold air moving freely on the other side. The compressed fill loses almost all of its insulating loft exactly where you need it most, so heat conducts straight out through the thin, flattened layer under your body.
This is a completely different problem from ground camping, which is why gear designed for tent camping often underperforms in a hammock even when the temperature rating on the label suggests it should be plenty warm.
Fix one: the underquilt
An underquilt is insulation that hangs beneath the hammock body rather than resting against it. Because it isn't compressed by your body weight, it keeps its full loft and insulating value all night. It attaches via straps or shock cord to the outside of the hammock, essentially wrapping the bottom half of the hammock in its own dedicated layer of warmth.
Advantages: full, uncompressed loft where you need it most; no cold gaps where a pad might shift; generally the warmest, most hammock-specific solution for a given weight.
Tradeoffs: hammock-specific gear that doesn't double as anything else; needs to be sized to your particular hammock's width and length; can be fiddly to attach correctly the first few times.
Fix two: an insulated sleeping pad
A sleeping pad works differently — instead of avoiding compression, it relies on materials (closed-cell foam, or insulated air chambers) that keep some insulating value even when compressed under your weight. Slide it inside the hammock, under your sleeping bag, the same way you would in a tent.
Advantages: doubles as ground-camping gear if you also tent camp; often cheaper than a dedicated underquilt of comparable warmth; simpler to set up, since there's nothing to strap to the outside of the hammock.
Tradeoffs: pads can shift around inside a hammock as you move, leaving a cold, uninsulated gap at exactly the wrong moment; a pad sized for ground camping may not match the width of a gathered-end hammock well, curling at the edges.
A quick fix while you decide: extra layers underneath
If you're caught between the two and want to test the waters before buying dedicated gear, a folded closed-cell foam pad or even a stuffed jacket placed under your hips and lower back can meaningfully reduce cold-butt syndrome on a mild night. It's not a long-term substitute for either proper option, but it's a reasonable way to confirm how much the compression problem is actually affecting you before spending money on a fix.
Which one to choose
If you hammock camp regularly and want the warmest, most reliable setup for the weight, an underquilt matched to your hammock's dimensions is generally the better long-term investment. If you split your time between tent and hammock camping, or you're not ready to commit to hammock-specific gear yet, a pad you already own (or a lightweight foam pad bought for double duty) is a perfectly workable starting point, especially in milder conditions.

The warmest option on paper isn't always the one that fits the trip you're actually packing for.
A worked weight comparison: closer than you'd expect
Weight is often assumed to be the deciding factor between the two options, so it's worth actually running the numbers rather than guessing. Adding a representative underquilt to a base hammock system, versus adding a representative insulated pad to the same base system, through the actual packed weight calculator function:

The pad option is lighter here, but not by nearly as much as the "underquilts are the heavy, dedicated option" reputation suggests — about 7 ounces, well under half a pound, and both land in the same weight class. In practice, the real-world weight gap between a well-chosen underquilt and a well-chosen pad is often smaller than the warmth, gap-free coverage, and convenience differences between them. If weight alone is pushing you toward a pad, run your own actual gear through the calculator before deciding — the numbers might be closer than the reputation implies.
Understanding R-value for the pad side of this decision
Insulated sleeping pads are commonly rated using an R-value scale, a standardized measure of resistance to heat flow that's used broadly across outdoor insulation products, not just pads. A higher R-value means more resistance to heat loss through the pad for a given thickness and material. When comparing pads for hammock use, a higher R-value is generally what you want for colder trips, but remember that this is a ground-camping-derived rating system — it doesn't account for the different way a pad behaves inside a hammock, where it can shift, curl at the edges of a gathered-end shape, or leave gaps in a way it wouldn't sandwiched between your body and solid ground. A pad's printed R-value is a genuinely useful comparison tool between pads, but it isn't a direct promise of how warm you'll be once that pad is inside a swinging fabric hammock instead of lying flat on the ground.
Sizing an underquilt so it actually does its job
An underquilt that's poorly matched to your hammock's width and length undercuts its own main advantage. Too narrow, and cold gaps open up along the edges exactly where a wider hammock's fabric curves away from your body. Too short, and your shoulders or feet end up outside its coverage on a hammock that's longer than the quilt was sized for. Most underquilts include adjustable suspension (straps, shock cord, or both) specifically so you can cinch the quilt up snug against the hammock's underside rather than leaving it hanging loose with air gaps between the quilt and the fabric — loose is a common first-time mistake, since a snug fit traps a thin layer of still air against the hammock body, which is a meaningful part of how the whole system stays warm.
Combining both for genuinely cold conditions
For serious cold, many experienced hammock campers don't choose one option over the other — they layer both. A foam or insulated pad inside the hammock, under a sleeping bag, combined with an underquilt on the outside, adds a wind and convection barrier on top of the underquilt's insulation, and the pad's presence also protects against the specific failure mode of an underquilt losing loft or shifting slightly out of position on a long, cold night. This isn't necessary for most three-season trips, but it's a legitimate, commonly used approach worth knowing about rather than assuming the choice is strictly either-or once temperatures drop seriously low.
Insulation has to follow a diagonal lay too
If you sleep diagonally across the hammock — the technique covered in sleeping flat on the diagonal, and genuinely the biggest single comfort upgrade available to most hammock campers — your insulation needs to cover you at that angle, not just down the centerline. An underquilt that wraps the whole hammock body generally handles this without extra thought. A pad, sized and centered for a straight lay, can leave a shoulder or hip hanging half off its edge once you rotate diagonally, recreating a cold gap in exactly the spot a pad is already somewhat prone to leaving one. If you're committed to a pad and you sleep diagonally, size up or reposition generously rather than assuming a pad cut for your hammock's length will automatically cover you at whatever angle you actually settle into.
A budget option worth knowing about
If you're not ready to commit to either dedicated option, a folded closed-cell foam pad layered with a thin reflective (space-blanket style) layer against your sleeping bag's underside is a genuinely workable stopgap for mild-to-cool nights, and it costs very little compared to either full option. It won't match a properly fitted underquilt or a good insulated pad in genuinely cold conditions, and it's bulkier per unit of warmth than either dedicated product, but it's a reasonable way to extend your comfortable range by ten or fifteen degrees without buying anything new, while you decide which dedicated option is worth the investment.
What actually decides it
Strip away the gear-shopping noise and the decision comes down to a short list of honest questions: do you hammock camp often enough to justify hammock-specific gear, or would a pad that also works for tent camping serve you better across more trips? Is the coldest night you're actually planning for mild, moderate, or genuinely cold, and does your current insulation plan actually match that rather than the average trip you usually take? And is weight the binding constraint on this particular trip, or is it, as the numbers above suggest, a smaller factor than it's often made out to be? Answer those honestly for your own situation and the underquilt-versus-pad choice mostly makes itself, rather than needing to be settled by a general rule that was never written with your specific trips in mind.
